Tree removal and tree pruning

I have an Elm tree that is 30-40 feet high. It seems to have Dutch Elm disease or another issue. Each late summer, it looses all of it leaves early, with the exception of the very top branches. This spring the leaves are coming in irregularly. I would love a quote for it to be treated.
